§ 3121.0310.
The department of job and family services shall adopt standard forms for support withholding
and deduction notices described in section 3121.03 of the Revised Code , which shall be used regardless of the type or source of income. All courts and child support enforcement agencies shall use the forms in issuing
withholding and deduction notices. The withholding and deduction requirements contained in the notices are final and
enforceable by the court.
