§ OC.604.103. APPLICATION
APPLICATION; APPLICATION FEE. An applicant for a certificate or temporary permit must: (1) apply to the advisory board on a form prescribed by and under rules adopted by the advisory board; and (2) submit a nonrefundable application fee with the application.
